HOW TO CLAIM YOUR PRIZE:
BC Children’s Hospital Foundation Choices Lottery 2012. BC Gaming Event License # 40415. Licensee: BC Children’s Hospital Foundation, 938 West 28th Avenue, Vancouver, BC, V5Z 4H4 Grand and Final Prizes drawn on May 3rd and May 4th, 2012.
All prize winners will be advised by phone and/or mail as to how to claim/receive their prize.
Any prize that has a cash option entitlement will not be available for pickup at the Prize Distribution Centre.
PLEASE NOTE: $100 LONDON DRUGS GIFT CARD, $100 SAVE-ON-FOODS GIFT CARD, $50 LONDON DRUGS GIFT CARD, $50 ESSO GAS CARD AND $50 TIM HORTON’S GIFT CARD WINNERS WILL HAVE THEIR PRIZE MAILED TO THEM AT THE ADDRESS ON THEIR TICKETS. These prizes are not available for pickup.
All Sunglasses, Bluetooth Plantronics Hands Free Visor Kit, Earthquake Kit, American Tourister by Samsonite Luggage, Delsey Deluxe Luggage, DeLonghi Espresso Maker and iPod Docking Station winners within the Designated Pickup Area, bounded by Abbotsford/Mission to the east, and Lions Bay to the north-and-west, excluding all islands, can pick up their prizes at the Prize Distribution Centre behind the Richmond Auto Mall at Crestwood Commerce Centre - 5600 Parkwood Way, Richmond, B.C. Please follow signs to free parking and temporary pickup site. You can pick up your prize as of Thursday, May 10th, 2012 to Saturday, May 26th, 2012, during the following hours: Monday through Saturdays, 9:30 a.m. to 4:30 p.m. Note: Closed on Sundays and the holiday Monday. Please bring one piece of photo identification together with your prize letter or one piece of photo identification together with your ticket.
Should a prize winner in the Designated Pickup Area not claim their prize by Saturday, May 26th, 2012 they will be advised of alternative delivery options by June 21st, 2012. Shipping and delivery charges will apply.
All other winners outside the Designated Pickup Area, east of Abbotsford/Mission and north-and west of Lions Bay on the North Shore, including all the islands, may pickup their prizes at the above temporary pickup site, or you will be mailed or couriered your prizes. You should receive them by June 21st, 2012.
Winners have been printed alphabetically by prize category, not in order of how they were drawn. Should there be a discrepancy in winning numbers; the records of KNV Chartered Accountants LLP will be deemed correct and final.
